feat: highlight visivo regole ineffective in pagine edit_ruleset

Aggiunge badge arancione 'ineffective' e bordo sinistro alle righe
marcate con icon_hyphen.svg (title='Ineffective rule'). Funziona sia
su wato.py diretto che dentro index.py con sidebar. Aggiunto helper
getTargetDoc() per gestire entrambi i casi. Attivazione via guard URL
mode=edit_ruleset.
